What is metastasis?

The organs of the body are surrounded by structures called basement membranes. These membranes are made up of proteins. When there is a breakdown of these proteins, there is metastasis. We now know from the sequencing of genes that tumors are made up of many different population groups. One of which is called the circulating cancer stem cell. These cells affect metastasis through multiple pathways via signaling molecules.

Metastasis to different sites in the body is influenced by the ability of the tumor to adapt to the organ microenvironment it ends up in. If it is successful there is a new colony of metastatic cancer cells. This metastatic colonization describes the last phase of metastasis. It is the establishment of a rapidly growing distant location of the cancer that has a new blood vessel supply to provide the cancer with nutrients and oxygen. This process can be very early, even before the primary cancer has been found. We can now tell you where your cancer may spread.

Metastasis Occurs Over a Period of Time

This can be broken down into several steps. The first is a mechanism called the “epithelial-mesenchymal transition (EMT).” These now mobile cells lose their polarity, change their cell shape, and have a destruction of their cell to cell junctions. Signals such as transforming growth factor-beta (TGF-beta), EGF, activate receptors such as the MAPK or PI3K pathway to trigger EMT and cancer spread. These cells enter the blood through lymphatic vessels and can spread to distant organs and reprogram them with new cancer cells. At remote sites, there is MET to cause a metastatic tumor.
These are now measurable via sophisticated molecular blood tests. These tests can reveal the genomic expression of these critical markers that can cause your metastasis. The only way to treat you is by analyzing these important bio markers.

The Pre-Metastatic Niche

The seed and soil theory is that metastasis only colonizes specifc organs because the spreading tumor cell has found a suitable environment to live in. This was first published by Kaplan and Oppenheimer in 2006. Studies have shown that factors released by the daughter tumor stimulate the spread of these premetastatic niche in select locations. I am now able to measure this in the laboratory.That is, we are able to determine where it is going to spread in each case.

By testing I can also develop specific immunotherapies and check point inhibitor drugs that will raise your immunity and can shut off your cancer. Because these new drugs have side effects, I only use them when indicated after testing to see if they’re effective.

Metastasis and tumor growth require new blood vessel growth called angiogenesis to feed the growing tumor. By choking the blood supply to the tumor and inhibiting new blood vessels we can starve the cancers blood supply. By testing we can find out how to choke off the blood vessels.
